Just provide your name, phone number and email, the date of the service you are interested in, and which service (morning or evening).

The CD will be ordered (allow 1-2 weeks). A donation of $1.00 is requested for the cost of the Cds.

When your Cd is ready, you can pick it up from the main reception office.
